Host MergeMe on prem [GitLab]

A few people have now requested the option to be able to self host MergeMe on their own infrastructure.

To me, it's a very valid feature especially for GitLab users who might already be used to self hosting.

For the initial release I am only doing this for GitLab. GitHub may be supported in the future if there is a want for it.
